The goal is to present a descriptive study related an unprecedent case of spinal myoclonus (SM) following subarachnoid anesthesia (SA). SM are sudden, brief, involuntary non-generalized spasms that can be an adverse effect of drug administration via neuraxial routes. Female, 67y, ASA II, proposed for hip replacement surgery, with normal preoperative exams. 7min after SA with 10mg of bupivacaine 0,5%, no motor blockade observed, and patient complained of unbearable pain in legs and perineum and bilateral, asymmetrical and arrhythmic myoclonic movements in the lower limbs. The latter solved after 48h of general anesthesia and rocuronium perfusion, amongst other therapeutics. Accordingly, intrathecal bupivacaine appears to be the SM most likely cause, regarding the absence of neurologic and electrolyte disorders, spinal cord direct trauma, drug exchange and normal perioperative examination, imaging and laboratory testing.It is mandatory to always take the patients' anaesthetic histories and recognize, treat and report rare anaesthetic complications.

BACKGROUND: In most countries, the amount of corneal graft tissue available for transplantation is insufficient to meet demand. In 2010, there were 459 patients on the waiting list for corneal transplantation (WLCT) in Espirito Santo (ES), a Brazilian state with 3,885,000 inhabitants. Several strategies were implemented to reduce the number of individuals on the WLCT, which reached zero in 2011. Studies on the evolution of corneal donation contribute to reducing the WLCT worldwide. The aim of this study was to describe the evolution of corneal donation in ES and the profile of corneal donors from 2010 to 2013. METHODS: This was a retrospective study. The data were collected from records referring to donations of corneas and from the archives of the Center for Organ Procurement and Distribution of ES. Statistical software was used to perform descriptive analysis of the data. RESULTS: The sample consisted of 1359 donors, aged 46.82 ± 17.32 (mean ± standard deviation), 70.1% of whom were male. Most of the consent forms for donation were signed by the sons of the donors. In 2011, there was a major increase (105%) in the number of corneal transplants compared with 2010; consequently, the number of patients on the WLCT reached zero, but increased again, reaching 139 patients in 2013. CONCLUSION: This study shows that strategies to increase corneal donations should be continued even after reduction or elimination waiting time on the WLCT.

The waiting list for corneal transplantation in Brazil had 6684 patients in June 2012, with great variations in the number of patients among different states. The mean waiting time for a corneal transplant also differs substantially across the states: it is 2 years in State of Espirito Santo (ES) at the time of this study. The aim of the study was to determine the main reasons for nonfulfillment of corneal donation in five ES hospitals. The data collection was performed from March to August 2010. The data were collected using forms completed daily by the Intrahospital Commissions of Organ and Tissues Donation for Transplantation. The total number of deaths was 1347. The main causes of nonfulfillment of donations were medical contraindications (67.86%), logistical/structural problems (27.84%), and family refusal (2.15%). The main medical contraindications were age outside the required range (24.49%), the presence of infectious disease (11.36%), or age outside the range plus infection (15.66%). In 2010, there were 459 people on the waiting list for corneas in ES. Considering the average of failures in these five hospitals multiplied by the number of hospitals in the ES, it seems clear that the solutions to these problems might eliminate the waiting list of the State. In fact, the number of individuals on the waiting list in ES did reach zero in August 2011, after the inauguration of a new Eye Bank that trains employees to interview and perform procurements, in addition to keeping them available in the most important hospitals. Additionally, the maximal age for donation in ES was increased.

Although, in absolute numbers, more organ transplantations are performed in Brazil than in many other countries, the donation rate (the number of donors per million individuals) remains quite low. The aim of this study was to determine the reasons for nonfulfillment of organ donations in five hospitals in the State of Espirito Santo, Brazil. This knowledge could help to identify strategies and implement changes to improve the organ donation process and to increase the rate in Brazil and other countries that face similar challenges. This prospective study collected data including the numbers of patients with suspected brain death (both confirmed and nonconfirmed), effective, and of nonfulfilled donations. Data were collected using forms completed daily by the Intra-Hospital Commission for Organ and Tissue Donation of the hospitals during the period from March to August 2010. There were 1386 deaths. The protocol for brain death diagnosis was initiated in 2.8% of them, but only 28.21% of confirmed brain death patients became effective donors. The causes of nondonation differed among the hospitals surveyed, suggesting that different strategies should be implemented to combat the specific weaknesses of each service, which must be continuously evaluated to increase the efficiency of the organ donation process.

Over the last five years, collection and periodic observation in the Tapacurá Ecological Station, São Lourenço da Mata, Pernambuco, Brazil, have been carried out to produce a systematic ecological inventory of grasshopper distribution within the station based on the classifying criteria for Acridoidea, providing new data on families, subfamilies, tribes, genera and species related to the various types of vegetation and environments the station affords.

The aim was to determine the effect of feeding and oral habits, toothbrushing, socioeconomic status and allergic rhinitis on the development of dental caries in primary dentition. In a cross-sectional study, data were obtained by means of a structured questionnaire on 1,160 children, 4-5 years old (mean = 4.5 +/- 0.5) and born in 1999, 2000 or 2001. The children also had a physical examination by an allergist and dentists. Dental caries was diagnosed according to WHO criteria. Caries experience was measured as number of deft and defs. Logistic regression analysis assessed the association between dental caries and independent variables. The dental caries prevalence was 17.9%, 28.8% of the children had allergic rhinitis symptoms, digit sucking was reported by 9.8% and pacifier use by 13.6% of the children. The mean number of deft of the sample was 1.02 (SD = 2.37) and that of defs was 1.33 (SD = 3.54). Analysis showed that breast-feeding for >12 months (p < 0.01), toothbrushing frequency (p < 0.01), sugar consumption (p < 0.01) and pacifier use (p < 0.01) each had a significant association with dental caries occurrence in primary dentition. Children with pacifier use and allergic rhinitis had more than double the risk of dental caries development. Allergic rhinitis alone has no effect on dental caries. Healthcare professionals attempting to limit dental caries should consider the effect of prolonged breast-feeding, sugary product consumption between meals and nonnutritive habits on dental caries.

Através de procedimento de coletas e observações periódicas, ao longo de cinco anos, na Estação Ecológica do Tapacurá, São Lourenço da Mata, PE, procedeu-se a um inventário sistemático-ecológico da distribuição dos gafanhotos da estação, adotando-se critérios classificatórios de Acridoidea com novos assinalamentos de famílias, subfamílias, tribos, gêneros e espécies, relacionados aos diversos tipos de vegetação e ambientes da estação.

The objectives of the present study were to optimize the protocol of mouse immunization with Paracoccidioides brasiliensis antigens (Rifkind's protocol) and to test the modulation effect of cyclophosphamide (Cy) on the delayed hypersensitivity response (DHR) of immunized animals. Experiments were carried out using one to four immunizing doses of either crude particulate P. brasiliensis antigen or yeast-cell antigen, followed by DHR test four or seven days after the last immunizing dose. The data demonstrated that an immunizing dose already elicited response; higher DHR indices were obtained with two or three immunizing doses; there were no differences between DHR indices of animals challenged four or seven days after the last dose. Overall the inoculation of two or three doses of the yeast-cell antigen, which is easier to prepare, and DHR test at day 4 simplify the original Rifkind's immunization protocol and shorten the duration of the experiments. The modulation effect of Cy on DHR was assayed with administration of 2.5, 20 and 100 mg/kg weight at seven day intervals starting from day 4 prior to the first immunizing dose. Only the treatment with 2.5 mg Cy increased the DHR indices. Treatment with 100 mg Cy inhibited the DHR, whereas 20 mg Cy did not affect the DHR indices. Results suggest an immunostimulating effect of low dose of Cy on the DHR of mice immunized with P. brasiliensis antigens.
